ADMISSIONS PROCESS

Thank you for your interest in Bridge Academy!

These registration materials must be returned to the school.

• Payment of Application Fee $150 USD (application fee is non-refundable)
• Application Form, completed and signed (all 4 pages, including medical information sheet and academic / psychological needs Information Sheet).
• Photocopy of Birth Certificate
• Photocopy of updated immunization records
• Psychological or psycho educational evaluation, if applicable
• Legal custody form, if applicable.
• Photocopy of Parent or Guardian’s picture ID (driver’s license, CIN, or Passport)
• One (1) recent picture ID of the student
• Report cards : last report of previous year and most recent report of current year

Please complete ALL information and check for parent’s signature where it is needed. All materials must be returned to complete the registration process. Please call the office at the number listed above or write an email if you have any questions regarding the registration process.
